What typically causes a boiler to become over-pressurised?

The most common reasons are: 1) the filling loop or filling key has accidentally been left open after topping up the system; 2) a failed or water-logged expansion vessel that can no longer absorb thermal expansion; 3) a faulty pressure-relief valve (PRV) that is stuck closed; 4) limescale or sludge blocking the system and trapping water in the heat exchanger; or 5) recent radiator bleeding without subsequently reducing the system pressure.
